Iowa's CURRENT QUALIFYING CONDITIONS
- Chronic pain
- Post-Traumatic Stress Disorder (PTSD)
- Cancer - which produces severe or chronic pain, nausea or severe nausea, and/or cachexia or severe wasting
- Crohn's Disease
- Ulcerative colitis
- Seizures
- Multiple sclerosis with sever and persistent muscle spasms
- AIDS or HIV as defined in Iowa Code, section 141A.1
- Parkinson's disease
- Amyotrophic lateral sclerosis (ALS)
- Corticobasal Degeneration
- Severe, intractable autism with self-injurious or aggressive behaviors
- Any terminal illness with probable life expectancy of under one year and producing: severe or chronic pain, nausea or severe nausea, and/or cachexia or severe wasting
IOWA MEDICAL CANNABIS CARD Certification
We are here to help certify patients for the Iowa Medical Cannabidiol Program, through tele-health or in-office consultations. Evaluations cost $100 and are scheduled for 30 min. Call or text us and we will get your appointment scheduled. All payments must be paid by cash, check or card (service fee for card usage), insurance is not accepted.
After the evaluation and meeting the qualifications for the Iowa Medical Cannabis Card, you will have to register with the Iowa Department of Public Health online or by mail. Required forms and additional information can by found at https://idph.iowa.gov/omc/For-Patients-and-Caregivers
